Activity Log

Log your success by reporting your School Key Contact events to the Indiana CPA Society.

Whether it is a job shadow experience, a networking workshop, a classroom presentation or a visit with a teacher or student, an activity log should be completed. The logs will be used to evaluate the School Key Contact program at every Indiana high school and gauge the type of involvement that is asked of our members.

The feedback received on these forms will allow the Society to gather data regarding the program, while giving the School Key Contact the ability to make suggestions about how the program can be improved. The feedback received from these logs is vital in improving the program.

All activity logs can be submitted at the end of each semester or at the conclusion of your event. Simply select the specific log that suits your recent communication with your assigned school and enter in the appropriate information that is requested. This data will be collected to assess the amounts and types of activities that are being coordinated at each school. Only one event should be reviewed per page.

At the completion of any high school event you are involved with, we would like you to complete the appropriate activity log (from the list below) in addition to the On Line School Key Contact Activity Log in order to document your participation accurately.
